USCIS Physical Exam for US Immigration

USCIS-designated immigration medical exams for Green Card and adjustment-of-status applicants. Complete and accurate Form I-693 documentation.

Travel Vaccinations & Pre-Travel Health

Official Yellow Fever and international travel vaccines. We provide preventative malaria prescriptions and consultations for your trip abroad.

Vaccination Fees & Information

TRAVEL VACCINES

    • Fee: $300

    • Recommended for eligible travelers 9 months and older traveling to areas with yellow fever risk or countries requiring vaccination.

    • A booster dose is required every 10 years.

    • People with severe egg allergies should discuss vaccine eligibility with the provider.

    • Lost vaccination certificate reissue: $60

    • Fee: $160

    • Injectable typhoid vaccine is available for people 2 years and older.

    • Our clinic recommends a booster dose every 10 years.

    • Booster recommended every 2 years if continued exposure risk remains.

    • Vaccination does not replace safe food and water precautions.

ROUTINE & PREVENTIVE VACCINES

    • Fee: $120 per dose

    • Clinic schedule: 3-dose series at 0, 1, and 6 months.

    • Protects against hepatitis B infection and related liver disease.

    • Other approved Hepatitis B schedules exist depending on the vaccine product and patient.

    • Fee: $120 per dose

    • Standard single-antigen series consists of 2 doses.

    • Doses are given at least 6 months apart.

    • Frequently recommended for travelers and others at increased risk of hepatitis A.

    • Fee: $80

    • Protects against tetanus, diphtheria, and pertussis (whooping cough).

    • Adults who have never received Tdap should receive one dose.

    • Afterward, a Td or Tdap booster is recommended every 10 years; Tdap is also recommended during each pregnancy.

    • Fee: $80 per dose

    • Helps protect against polio, a viral disease that can cause paralysis and serious neurological complications.

    • For unvaccinated adults, given as a 3-dose series.

    • Dose 2: 1–2 months after Dose 1

    • Dose 3: 6–12 months after Dose 2

    • Standard Flu Vaccine: $50

    • High-Dose Flu Vaccine / Flublok: $80

    • CDC recommends flu vaccination every year for everyone 6 months and older, with rare exceptions.

    • Annual vaccination is needed because protection declines and circulating flu viruses change.

    • No new-patient registration fee for flu vaccination.

CERTIFICATES & OTHER SERVICES

    • Fee: $120

    • DOT physical exam for commercial drivers.

    • Performed by an FMCSA-certified medical examiner.

    • Fee: $150

    • Immigration medical exam by a USCIS-designated civil surgeon.

    • Form I-693 completion included as required.
